D12.5W through peripheral IVs Standard dextrose concentrations in peripheral IV fluids are D7.5W and D10W. It is preferred that D12.5W be administered in central TPN. However, if a baby with only peripheral IV access requires D12.5W to prevent hypoglycemia, D12.5W can be given through a PIV. There is no time limit for giving D12.5W peripherally. However, Ca++ should be omitted from D12.5W solutions being infused peripherally. D12.5W alone is safe to administer through PIVs; but due to increased osmolarity, peripheral usage of D12.5W may shorten the useful lifespan of the IV. The addition of Ca++ not only increases the osmolarity of the solution as a whole, it certainly increases the risk of significant injury caused by an accidental IV infiltrate. Nurses should report IV infiltrates as per protocol and include in the details of the report the dextrose concentration and calcium content of the offending IVF.
