Standard Drip Concentrations and Rate Calculation

 

The NICU promotes the use of standardized concentrations for all continuous infusions. Drips may only be ordered via the computerized order entry system (WIZ/HEO) or pre-printed order sheet available for patients prior to admission.  JCAHO prohibits the use of customized drip concentrations and the Rule of Six for calculating drip infusion rates is no longer accepted.


To order drips for an unborn or outborn patient not yet entered in the Vanderbilt computer system, a pre-printed order sheet is available that assists with calculations.

In an attempt to standardize the formulation of these medications for continuous infusion, the pharmacy requests (and WIZ/HEO pages require) that drips be ordered in either D5W, D10W or NS if compatible. The small volumes in which these drips are administered to neonatal patients preclude providing a significant increase in a patient’s caloric intake by using a higher glucose concentration.

 

Rate calculations are available through HEO and are patient specific (see example below).  This chart may be displayed at the bedside for easy access and reference.

1.      Enter patient name

2.      Click on Look Up Orders

3.      Click on Medication Order (ie dopamine, dobutamine, etc)

4.      Click on View Printable Drip Chart

5.      Click on Print
